Transforming Clinical Data into Actionable Patient Summaries

About Connective Health 

Connective Health, a data interoperability company, utilizes automated medical record retrieval and artificial intelligence (AI) to provide primary care and virtual specialty care providers with medical summaries and clinical insights. Detailed patient summaries are developed by analyzing clinical records from various sources, including patient indexes, record location databases, electronic health records (EHRs), pharmacy databases, and state admission, discharge, and transfer (ADT) notifications. Connective Health’s solution ensures providers have quick and easy access to tailored patient data and insights in their practice management or EHR system to make effective outreach and care delivery decisions. Each month, Connective Health engages with approximately 4,000 clinic sites and analyzes one million clinical documents.

Challenge

Providers are often left in the dark about critical patient information, one study showing that medical records are consistently missing 30% of a patient’s chronic diagnoses, which creates an incomplete picture of chronic disease burden, utilization, risk, and more. This can lead to incorrect or wasteful use of healthcare resources, avoidable cost and burden for the provider, and poor health outcomes for the patient. To close the information gap, Connective Health pursued the vision of acquiring and aggregating multiple streams of clinical data from various sources to create a single longitudinal record for providers. This would provide clinicians with information from in and out of network encounters and various care settings to ensure a complete patient history could be used to predict future risks, manage conditions, and align resources to patient needs. 

Despite Connective Health’s advanced automated medical record retrieval and AI-driven systems, ensuring the quality of data in patient summaries remained a challenge. This is due to the wide variability in data quality and consistency from various sources. Issues such as duplicate records, outdated information, inconsistent data formats, missing context, incorrect entries, lack of standardization, and duplications can lead to incomplete or incorrect patient summaries, impacting clinical decision-making and patient care. To address these challenges, Connective Health sought a clinical data quality improvement vendor with the technology and expertise to transform multi-source and multi-format clinical data into a standardized and structured format that can then be used to enhance the reliability of patient summaries, improve clinical insight accuracy, and drive optimal patient outcomes.

Solution

Connective Health partnered with Availity to integrate Availity Fusion™, an automated data transformation engine designed to standardize, enhance, and optimize clinical data from multiple sources and formats, into their patient summaries solution. Connective Health’s advanced interoperability service retrieves approximately 300,000 medical documents per week from around 1,000 different healthcare sites, which are then processed by Availity Fusion in real-time. Availity Fusion’s API-based technology engine ingests and parses the clinical data, normalizes it to national standards, enriches it with metadata for efficient analytics, reorganizes data into expected clinical sections, and eliminates redundancy by deduplicating and summarizing information. 

After Availity Fusion improves the quality of clinical data, Connective Health applies advanced data aggregation and intelligence to generate concise, one-page patient summaries with key clinical insights. These summaries are then integrated into providers’ existing workflows, allowing easy access to recent updates to a patient’s medical history, optimizing new patient intake and triage, enhancing risk adjustment and quality gap closure, and supporting care management with tailored clinical content.

Results

In a recent study, Connective Health partnered with a provider group to engage ~2,000 patients with more than 3,000 open care gaps, due to multiple patients having more than one quality gap. Connective Health’s care summaries provided supporting documentation to close 32% of those open gaps – reducing the need for provider outreach on already closed gaps and, thus, preventing duplicate outreach, wasted resource time, and poor patient experience. With an updated gap worklist to operate from, the providers then focused efforts on engaging patients around the remaining gaps and scheduling visits to address appropriate conditions, administer preventative screenings and immunizations, perform medication adherence, and deliver high-quality care. 

Additionally, providers using Connective Health’s patient summaries have increased the success rate of triaging patients into specialty programs due to the availability of complete, accurate information on chronic diseases. Specialist providers, who depend on sufficient medical history to treat patients, are now able to start treating 5% more patients due to efficiency gains from the summarized data from Connective Health. These workflow enhancements are made possible by the critical combination of Availity Fusion’s high-quality data output and the comprehensive, curated medical information provided into the provider workflow by Connective Health.

Availity Fusion has successfully upcycled and processed over one million clinical documents per month for Connective Health, ensuring comprehensive and accurate patient data without delays or workflow interruption.

Streamlined Data Presentation

On average, Connective Health ingests 100 pages of records to provide physicians with a concise one-page patient care summary, making critical information easily accessible in a clean, useful format.

Identification of Undiagnosed Conditions

Utilizing Availity Fusion’s automated data transformation engine, Connective Health identifies critical missing information, such as missing chronic or severe recent acute diagnoses for 35% of patients’, and presents summarized information to support these findings.

Closing Gaps in Care

By delivering high-quality medical information and more complete data for reporting, Connective Health helps providers close a significant amount of quality care gaps. This allows providers to concentrate on targeted patient outreach for remaining open care gaps, leading to more efficient resource utilization and enhanced patient experience and outcomes.

Time and Cost Savings for Providers Pre-Visit

By utilizing Connective Health’s summary records, providers and care managers could experience a 50% reduction in the time spent gathering outside records and reduce the timeframe from two weeks to less than an hour, resulting in significant cost savings and less patient frustration. This efficiency gain comes from reduced time spent requesting records, sifting through extensive medical documents, and manually reconciling information from multiple sources and encounters.

By combining Availity Fusion’s automated data transformation engine and Connective Health’s tailored patient summaries, Connective Health can deliver an end-to-end clinical data acquisition, quality improvement, and point of care insights solution that closes the information gap to ensure providers make the most informed decisions. The outcome is that providers are now able to access complete, high-quality information on their patients – all within their EHR system – to drive efficient pre-visit planning, care delivery, and follow-up activities.
